About Domanico Consulting

About Anthony Domanico.
I've been on both sides of the table.

Roughly a decade inside health systems. Another decade in consulting. A perspective built from understanding the details of the work and seeing how other organizations approach it.

The perspective

Inside the work. Across the industry.

For roughly a decade I worked inside health systems, responsible for the physician compensation and operations that make a program function. That's where you learn that a decision doesn't end when it's approved. It has to survive the people, the processes, and the budgets that carry it.

Another decade in consulting gave me the other view: how different organizations approach the same problems, each with its own structures, governance, and constraints. No two solve it the same way, and most of what looks unique has a version somewhere else.

The combination is the point. I understand the details of one organization's reality, and I've seen enough of the broader pattern to know which parts actually matter.

What clients get

Experience that travels both ways.

The value isn't time spent in a chair. It's knowing what a decision looks like from the inside, and what it looks like after seeing it made a hundred different ways.

01

Operational depth

An understanding of how a compensation decision affects the people and processes responsible for making it work.

02

A broader perspective

Exposure to how other health systems approach similar compensation and operational challenges — without assuming yours should be solved the same way.

03

Practical judgment

Recommendations grounded in what is workable inside your organization, not just what looks defensible on paper.

04

Continuity

A partner who can carry context forward instead of starting from zero with every question that comes up.

Why Domanico Consulting

Good advice should make the work easier.

Having worked both inside health systems and in consulting, I know the difference between a recommendation that sounds right and one that can actually be put into practice. Domanico Consulting is built around that distinction.

The goal is to bring experienced provider compensation judgment to your decisions, make them easier to understand, and work as an extension of your team — without standing up the usual consulting machinery around it.

In addition to managing compensation portfolios and leading teams, I built a $7M ARR consulting practice from the ground up — which is also how I learned what most consulting engagements get wrong.

Bring the question. Start with a conversation.

Whether you need a second opinion on one compensation decision or a more consistent partner for the questions that keep coming up, the starting point is the same.
